FOR ALL PATIENTS

Before your first appointment, (or a later appointment if you are attending for a new problem), you should receive an email from us, containing a link for you to complete some forms that are necessary to assist your physiotherapist in planning your assessment and treatment. Don’t worry if you do not have access to a computer or if you forget to complete them before you arrive, because you will have an opportunity to complete them in the clinic.

PLEASE NOTE, HOWEVER, THAT IF YOU ARE UNABLE TO COMPLETE THEM BEFORE YOU ATTEND YOU WILL NEED TO ARRIVE APPROXIMATELY 20 MINUTES BEFORE THE TIME OF YOUR SCHEDULED APPOINTMENT. This will allow you time to complete these forms. If you have questions about what time you should arrive please contact our reception staff and they will clarify this for you. Please note that you arrive late and haven’t completed the forms by the time of your appointment, this may mean that your physiotherapist will have less time to spend with you before the next patient is scheduled.
